Not sure about your plan?

Find a provider who accepts your insurance
- Tell us your plan and preferences. Use the prospective client form to share your insurance information, the age of the person seeking care, whether you prefer in-person or telehealth visits, and any provider preference.
- Confirm the provider and coverage. Our client care team can help identify participating providers. Confirm your benefits and expected costs with your insurer and our team before the first appointment.
- Choose your scheduling option. Selected therapy providers offer online scheduling for clients ages 12 and older, with the first visit by telehealth. If your provider is not listed online or you need another appointment format, use the prospective client form.
Meet our therapy team or learn about individual therapy before choosing a provider.

Creekside has providers who participate in Medicaid. Our client care team can help confirm options for your specific coverage and the care you are seeking. Not every provider accepts every plan.
Medicaid clients meet with an intake clinician before their first appointment with their ongoing therapist or prescriber. Complete the prospective client form — opens in a new tab so our team can help arrange this process.
Common insurance questions
Does every Creekside provider accept my plan?
No. The list above shows plans accepted by participating providers across the practice. It does not mean every provider accepts each plan. Ask our client care team about the provider and service you are considering.
What should I ask before my first appointment?
Ask whether the specific provider participates in your plan, whether your requested service and appointment format are covered, what you may need to pay, and whether any referral or authorization is required. A plan appearing on this page does not guarantee coverage for a particular visit.
What if I cannot find my provider in online scheduling?
Only selected providers offer online scheduling. Complete the prospective client form to ask about other providers, appointments for children younger than 12, or an in-person first visit. You can also call (303) 770-6933 for help.
ANOTHER WAY TO PAY
