Frequently asked questions

Clear answers about scheduling, appointments, insurance, and your care at Creekside.

Getting started

What services do you offer?

Creekside offers individual, family, and group therapy, along with psychiatric medication management. Services, age groups, and availability vary by provider.

You can receive therapy, medication management, or both. You do not need to receive both services at Creekside. When you see more than one provider, they can discuss coordinating your care with you.

How do I schedule my first appointment?

Start with Schedule Appointment. You can choose the option that works best for you:

  1. Book online: Our preferred option when an eligible provider and appointment are available.
  2. Complete the prospective client form: If you cannot book online or prefer help finding a provider, send our client care team a prospective client form — opens in a new tab.
  3. Call us: Reach our client care team at (303) 770-6933.
  4. Send a message: Email creekside@creeksidecollaborativetherapy.net or use our contact form — opens in a new tab.

These are alternatives; you do not need to complete every step. If a provider’s profile gives specific scheduling instructions, please follow those.

Can I book every provider or service online?

Online booking is available for selected therapy providers serving clients ages 12 and older. First appointments booked online are by telehealth.

For medication management, an in-person first appointment, a younger client, or a provider who is not available online, please use the prospective client form — opens in a new tab. Our team can help you explore available options.

How will you help me find a provider?

Our client care team considers your age group, the concerns you would like help with, your insurance if you plan to use it, and provider availability. You can also review provider profiles to learn about their approaches and experience.

Is the intake process different if I have Medicaid?

Yes. Medicaid clients meet with an intake clinician before their first appointment with their ongoing therapist or prescriber. The intake clinician shares the assessment with your Creekside provider to help guide treatment planning and goals. Our client care team will explain and help arrange these appointments.

What paperwork do I need to complete?

Our client care team sends intake paperwork through IntakeQ, our online forms system. Forms may include an intake questionnaire, client registration, practice agreement, privacy acknowledgment, telehealth consent, electronic communications consent, and provider-specific disclosures. A credit card form is generally included for clients who do not have Medicaid.

Please complete the forms sent to you before your first appointment. If you cannot complete them online, let our team know so we can arrange another way. You can also review our Notices & Forms.

Appointments & care

Do you offer telehealth and in-person appointments?

Yes. Creekside offers telehealth and in-person appointments. Options depend on the provider and service. Your provider will send the details needed to join a telehealth visit.

First therapy appointments booked online are by telehealth. If you need your first appointment to be in person, please use the prospective client form — opens in a new tab.

How long are appointments?

Appointment length depends on your provider, the service, and whether it is your first visit or a follow-up.

  • Therapy: Generally 45–60 minutes.
  • Initial medication management: Generally 40–60 minutes.
  • Medication management follow-up: Generally 20–30 minutes.

Confirm the length of your specific appointment when scheduling.

What is your cancellation and missed-appointment policy?

Please give at least 24 hours’ notice for therapy appointments and 48 hours’ notice for medication management appointments. Weekends and holidays do not count toward the notice period. Contact your provider or our client care team as early as possible if you need to cancel or reschedule.

Late cancellations and missed appointments may result in an appointment charge for clients who do not have Medicaid. Medicaid clients are not charged these fees. Your provider can take emergencies into account. Repeated late cancellations or missed appointments may affect your ability to continue with that provider.

Please review your provider’s disclosure and agreement for the applicable fee and policy details. If you are unsure of your cancellation deadline, ask your provider or call (303) 770-6933.

Can my provider coordinate with professionals outside Creekside?

Yes. Creekside values collaboration. Discuss coordination with your provider, including which professionals you would like involved. Your provider will explain any authorization needed before sharing information. A Release of Information form — PDF opens in a new tab is available on our Notices & Forms page.

Where is your office?

Our office is at 6000 Greenwood Plaza Blvd #105, Greenwood Village, CO 80111.

Confirm whether your visit is in person or by telehealth before coming to the office. Call (303) 770-6933 if you need help.

Insurance & payments

Which insurance plans do you accept?

Visit our Insurance page for the current list of plans and networks accepted by participating Creekside providers.

Not every provider accepts every plan. Our client care team can help identify participating providers. Please confirm your specific benefits, coverage, and any out-of-pocket costs with your insurer and our team before your appointment.

Can I pay without insurance?

Yes. Self-pay is available. Contact our client care team to discuss rates for your provider and service. If you need a reduced-fee option, ask whether one is available; options and availability vary.

Read about your right to a Good Faith Estimate when you are uninsured or paying without insurance.

How do I add or update my payment information?

Use our existing payment information form — opens IntakeQ in a new tab. It opens IntakeQ and asks for your name and email to begin.

Please do not send credit card details through ordinary email or our general contact form. If you need help accessing the payment form, call (303) 770-6933.

Privacy, records & feedback

Where can I read your privacy notice?

Our Notices & Forms page includes the Notice of Privacy Practices, practice disclosure and agreement, and Release of Information form.

How do I request my medical records?

Visit Medical records for instructions and the online records-request form. If you need help or another way to request records, call our client care team at (303) 770-6933.

How can I share feedback or raise a concern?

You can discuss a concern with your provider or use the grievance and feedback resources on our Notices & Forms page. That section includes separate forms for submitting a grievance and sharing feedback about your provider.
